Wellbeing and Prevention Support Worker
Scarborough, Whitby and Ryedale
30 hours per week
Flexible over 4 - 5 days a week.
Salary £16,167 - for 30 hours (pro rata £19,939)
You will be joining our experienced team working on a new service supporting adults 18+ (predominately focusing on older people and people with physical disabilities) to stay well and independent in their homes and local communities. The role of the Wellbeing and Prevention team will be to prevent, reduce and delay the escalation of health issues, placing unnecessary demand for long-term statutory care and support.
You will need to be experienced in completing initial assessments and action plans to support this client group. This is a needs-led service which will require you to be proactive and innovative in your approach to supporting clients who are at risk of deteriorating health, social isolation and loneliness.
You will be required to link closely with our partner organisation (Ryedale Carers Support) to develop and promote the service, as well as identify and support with various activities. Good communication skills will be required. You will have experience of working with people in their own homes and in the community.
Flexibility is a requirement of the role. Own transport essential.
